Although you definitely seem to have some severe mental health issues, I seriously doubt that you are certifiable enough to be involuntarily committed.
You have already taken the first step in realizing that you have mental health problems for which you need treatment. Don't let your fears prevent you from getting it.
If you find that you cannot push yourself to take the first step, do you feel comfortable confiding in someone and asking that person to act as your support and backup person? Perhaps that person would be willing to go with you and wait while you complete your appointments until such a time as you feel comfortable going by yourself.
If you don't think you can confide in anyone, I suppose you could always try the self-help book route, but I think that might be a bit difficult based on the disorders you believe yourself to have.
