This concerns Medicare and a letter sent to me from WALMART STORE TEX.
I've lost the letter and I'm trying to find where I can get another copy.
Walmart, via Medicare, has now REQUIRED that I send them my GLUCOSE MONITOR READINGS FOR SIX WEEKS EVERY SIX MONTHS.
They had sent forms for me to send, however, I misplaced the letter with the forms. I've been to Medicare, to Social Security, to Walmart (on the phone and on the web).
A gentleman, ON THE PHONE, with Mecare gave me the above address, but he said he didn't know the phone number.
I've searched until I'm blue in the face. Can ANYONE help me with this? I don't want to get kicked off of Medicare because of this ONE glitch of mine in nine years.
