Everything I say can be fully substantiated by my own opinion.

Probably because there was more travel available in 1918 than there was in the 14th century.  The infected were spreading the flu worldwide during WWI.

Also, people in the 20th century had more hygienic lifestyles, which reduces the immunity abilities of the body.  The reason Polio became prevalent in the early 1900s was because people were no longer exposed to as many germs and had no resistance built up.

Firstly, because the population density was lower when the plague struck.

Secondly, because the plague was limited to part of Europe, while the Spanish flu was a world wide epidemic.
